i ~ <br />(Page 3) <br />MINE ID # OR PROSPECTING ID # M-82-079 <br />INSPECTION DATE 6/11/98 INSPECTOR'S INITIALS AJW <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There was not a legible mine identification sign posted 08/31/98 <br />at the public entrance to the mine site as required by PB or PV: PB <br />Rule 3.1.12 (1)(al, (b) & (c), and the affected area TOPIC(S): SM, <br />boundaries were not fully marked by monuments or other RS <br />markers as required by Rule 3.1.12(2) <br />Corrective actions are to: <br />1) Place a mine identification sign at <br />the entrance to the mine site that <br />contains at a minium: the name of the <br />operator and "MLRB Permit # M-82- <br />079". <br />2) Mark the boundaries of the affected <br />area with markers that are clearly <br />visible and adequate to delineate <br />such boundaries. <br />3) Submit a photograph to our office <br />which clearly shows that the <br />identification sign and affected area <br />boundary markers have been installed. <br />2. The permit application did not contain a map showing 8/31/98 <br />information sufficient to determine the location of the PV or PB: PB <br />affected land boundaries as required by Rule 6.3.5(2). TOPICS: SM, RS <br />Corrective AcCions are Co: <br />Provide a map that meets the requirements of Rule 6.3.5 <br />and at a minimum is of sufficient detail to clearly <br />identify the permiC area boundaries so that the permit <br />area can be accurately marked and verified on the <br />ground. <br />
