(Page 2)
MINE ID # OR PROSPECTING ID # M-81-2'76
INSPECTION DATE 3/1/2000

INSPECTOR'S INITIALS TAS
1. A site inspection was made of the Hertzog Pit. The inspection was made as a follow-up to
the Division's previous inspection on 12/16/99. Present during the site inspection was Mr.
Wes Hertzog.
2. An entrance sign and permit boundary markers have been posted as required by Rule 3.1.12.
This concern as noted in the previous inspection report has been adequately resolved.
3. At the time of inspection, the operator hand delivered a completed survey of the entire
disturbance at the Hertzog Pit. The total disturbance is noted to be 34 acres and occurs
well within the boundaries of the 85 acre permit boundary. However, the existing mine plan
specifies a maximum disturbance at any one time of 10 acres, of which 5 acres may be active
pit disturbance. The operator is currently in the process of revising the mine plan, via
technical revision, Co increase the maximum allowable disturbance at any ona time. Also, the
reclamation cost and required financial warranty will be reevaluated in accordance with the
current disturbance and desired maximum allowable disturbance. The Division considers this
matter to be a problem that is currently being addressed by the operator.
