Boa 774018 <br />Steamboat Springs, CO 80477 <br />Havden Gulch Mine <br />June 4, 2002 <br />Complete inspection <br />Participants: <br />John Weinman, Montgomery Watson <br />Jim Stark, Colorado Division of Minerals and Geology (DMG) <br />Henry Austin, Office of Surface Mining (OSM) <br />I participated in a complete inspection of the Hayden Gulch Mine on June 4, 2002, with <br />the above. This mine was selected for inspection by the joint DMG / OSM Oversight <br />Team. Weather conditions were warm and partly cloudy, and ground conditions were dry <br />throughout the areas inspected. <br />Mr. Stark began the inspection by conducting a thorough permitting and environmental <br />monitoring records review at the Montgomery Watson offices. We reviewed records <br />including the following: current surety bond liability is with Aetna Casualty in the <br />amount of $27,437.27; Phase lI and III bond releases were approved at [his site during <br />April 1999, and Technical Revision No. 62 describes the three shrub plots which remain <br />to be released at the site. These shrub plots, and the 1994 slump area are the only <br />remaining reclamation areas currently being evaluated and awaiting final Phase II and III <br />bond release. The mid-term permit review was completed August 11, 1999, and currently <br />there is an application for permit transfer to Peabody Coal Company being reviewed by <br />DMG. The iVPDES permit was terminated October 29, 1999. The required liability <br />insurance certificate is in the process of being renewed, and the Annual Report to DMG <br />has been submitted as required. This completed the records review and all required <br />reports reviewed were in compliance. <br />~rJe began the field inspection at the former pond 3 that has been retained as a permanent <br />water impoundment. Adjacent and upslope of this impoundment is one of the 3 shrub <br />clumps established on the permit area. This area appeared to be the least successful of the <br />3 clumps reviewed. The area was stable and well vegetated with mountain brome <br />primarily, and shrubs are becoming established. All 3 clumps are fenced with elk proof <br />type fence. The second clump reviewed is contiguous to former pond HG-8 that has also <br />been retained as a permanent impoundment. Again the area is stable, with good <br />vesetative cover, and shrubs are becoming established. The seeps on the north facing <br />slopes to the pond drainage area were not very active, one was wet but didn't appear to be <br />flowing. <br />
