<br />(Pale 4) <br />MINE ID # OR PROSPECTING ID # M-99-034 <br />INSPECTION DATE 12/04/00 <br />INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLH VIOLATIONS <br />AND <br />CORRECTIVH ACTIONS <br />DATE <br />1. The Division was not present on site during the construction of the _ <br />slurry wall, making it impossible to determine if the mining PB or PV:PB <br />activity caused the well to collapse. Therefore, the operator has TOPIC(S):HB <br />the following options; January 12, 2001 <br />• Provide the Division a description of the site activity during <br />construction of the slurry wall and describe how the <br />activities have or have not resulted in hydrologic impacts to <br />Dorothy Ford's well. In addition, please provide the status <br />of the Office of the State Engineer letter to the Division <br />dated May 17, 1999 regarding wells within 600 feet of the <br />permit area (letter attached). OR <br />• Resolve this issue by working with the landowner to ensure <br />that the water rights are protected. <br />A written response is required by January 12, 2001. <br />~~ <br />
