Rollings, 370, OSM <br />Susan Burgmaier, DMG <br />Kent Gorham, DMG <br />Larry Routten, DMG <br />Chuck McCulloh, PCC <br />This was a random sample, oversight inspection conducted with the state RA and <br />PCC representatives. The last Federal inspection was on 9/26/91. The last DMG <br />complete inspection was on 6/16/93. <br />Records and permits appeared to be in order, other than the blasting records <br />discussed below. NPDES # CO 0000221 was issued 5/27/92 and expires 5/31/97. <br />Liability insurance is with National Union Fire Ins., and expires 10/1/93. Pond 005 <br />was certified 8/17/92, pond 006 on .12/12/91, and pond.009.on 7/9/92. The last .. <br />quarterly inspection was performed on 6/10/93. PCC has a four million dollar <br />reclamation bond in place for this site. The following is a list of certified roads and <br />the dates of certification: road F - 9/12/91, tie across haul road - 2/28/91, mine <br />entrance road - 11/5/90, road A - 11/5/90, and road G - 9/12/91. The most recent <br />blasting notice was published for the effective period 10/1/92 through 9/31/93. <br />Enforcement Actions <br />DMG issued three enforcement actions, #'s C93-133 through 135, as a result of <br />this inspection. An NOV was issued for the topsoil pile identified as OO. The pile <br />is not addressed as an SAE like most of the others, and the runoff from the pile <br />would not pass through a siltation structure. OO is not identified on the maps as <br />an SAE either, so the runoff from this pile is required to be passed through a <br />siltation structure. <br />An NOV was issued for the area near CMP location A3, along road A, for the <br />sediment that has accumulated from the outlet of the CMP to a point about 150 <br />yards off of the permit area. The sediment is from the road disturbance only, not <br />from the mining area. PCC is using coal during the winter to surtace their roads <br />
