(Page 2) MINE ID # OR PROSPECTING ID #: ~-~4e5_mn INSPECTION DATE: n~r19-/m INSPECTOR=S INITIALS: KAP OBSERVATIONS This inspection was conducted as part of the Division of Minerals and Geology's monitoring program. Present at the inspection were Kate Pickford, DMG and Harry Corkle, owner/operator. Weather at the time of the inspection was clear, calm, and approximately 75 degrees. 2. A permit sign was posted at the entrance to the permit area, and the boundaries of the permit site were marked. All previous mining activity at the site appears to have occurred within the permit boundary. Topography of the site indicates that any surface water runoff would be contained on site. There was no evidence of weeds on the site. 3. The mine plan indicates that a slope of 3:1 or less will be maintained during extraction of material except where material runs deeper. There are sections of the site which have slopes of nearly l :l consisting of a highwall approximately 150 feet long and 8 feet high which would have to be graded, and two highwalls approximately 450 feet long and10 feet to 18 feet high, one of which could be graded, the other would have to be backfilled due to it's location immediately adjacent to the boundary fence of the site. Considering the grading and backfilling required to reclaim these highwalls, the operation is still well within its bond amount. 4. A topsoil stockpile is located along the north boundary of the western portion of the site. There appears to be adequate topsoil for the site, however, the stockpile is not vegetated. 5. An above ground storage tank containing diesel fuel is located along the western edge of the site. There is to containment structure to contain the contents of the tank in the event of a spill or leak.
