The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMSIPOSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />I. The mining sequence does not follow the mining plan. 9/16/94 <br />PB Or PV PB <br />Corrective Action: Submit a map and revised mining plan TOPIC(S) AR.MP <br />in the form of a revision to the permit no later than <br />9/16/94. <br />2. A stockpile of sediment pond fines has spilled down the 9/9/94 <br />northeastern side of the permit and onto the banks of PB or PV PB <br />the Middle Fork of the South Platte River. TOPIC(S) PW <br />Corrective action: Pull back the remainder of the <br />sediment pond fines still stockpiled along the edge of <br />the slope and create a berm along the edge of the slope <br />to prevent any material from washing down toward the <br />river. Spread the fine material along the river bank <br />into the low area to the west and create a three foot <br />berm along the river edge to prevent any fines from <br />entering the river. Install berms along the path of the <br />slide to hold back any material still suspended along <br />the slope. <br />3. All permit boundaries were not clearly marked and a 9/9/94 <br />sign was not observed at the entrance. PB or PV PB <br /> TOPIC(S) SM <br />Corrective action: Post the appropriate sign and <br />boundary markers pursuant to Rule 3.1.12 no later than <br />9/9/94. <br />4. Three drums of waste oil were observed in the facility 9/9/94 <br />area. PB or PV BP <br /> TOPIC(S) PW <br />Corrective action: Properly dispose of the waste oil <br />and provide proof (bill of lading) to the Division no <br />later than 9/9/94. <br />5. There is no stormwater permit for this operation. 9/31/94 <br /> PB or PV PB <br />Corrective Action: Submit verification of application TOPIC(S) SP <br />for a stormwater management plan to the Division no <br />later than 9/31/94. <br />
