<br />The following problems (PB) and/or possible violations (PV) (and <br />suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, <br />or they will become possible violations. The possible violations <br />should be corrected by the dates given to reduce their severity <br />when considered by the Mined Land Reclamation Board. The inspector <br />noted on the previous page should be notified of all corrective <br />actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There was not a mine identification sign <br />posted at the public entrance to the mine <br />site as required by Rule 3.1.12(1)(a),(b) & <br />(c), and the affected area boundaries were <br />not marked by monuments or other markers as <br />required by Rule 3.1.12(2) <br />Corrective actions are to: <br />1) Place a mine identification sign at the <br />entrance to the mine site that contains <br />at a minium: the name of the operator <br />and "MLRB Permit # M-99-003". <br />2) Mark the boundaries of the affected area <br />with markers that are clearly visible <br />and adequate to delineate such <br />boundaries. <br />3) Submit a photograph to our office which <br />clearly shows that the identification <br />sign and affected area boundary markers <br />have been installed. <br />07/15/99 <br />PB or PV: PB <br />TOPIC(S): SM <br />
