• (Page 2) •
MINE ID # OR PROSPECTING ID # M-78-056
INSPECTION DATE 09/23/96 INSPECTOR'S INITIALS CLK
OBSERVATIONS
The following observations were made during a routine inspection of the 164 acre 112 Bite.
1. While the boundaries are marked by fences and the canal, a sign with the permit number
and operation name moat be posted at the entrance within 30 days.
2. Concrete is being stored and recycled on the site. A technical revision to the permit to
account for this must be submitted within 30 days.
3. 10,725 CY of topsoil is stored more than 1000 ft. away from the main excavation (enough
to reclaim 13.5 acres) and topsoil also lines the edges of the elopes above the pit, in
quantities large enough to reclaim Phase SA & l0A of the site, but not phases 8A and 9A
(which make up 30.3 acres). There is no topsoil deficit, if we are assuming that the
original site plan of leaving only 35.4 acres as dry land ie adhered to.
4. Approximately 65 acres is currently disturbed, with the depth of the mining reaching 27
feet deep. Final reclamation has not been attempted on the site, although approximately 100
acres remain undisturbed. It seems highly likely that the currently posted bond of $6,000.00
ie inadequate to cover the coat of site reclamation at this time, especially with the
majority of the topsoil stored ae far away from the excavation ae it is, and the addition of
the concrete recycle plant. The bond for this site will recalculated within a month, when,
if it is necessary, a request for an increase to the posted financial warranty will be made.
No other problems noted at this time.
I & E Contact Address
NAME Al Lunberrv
OPERATOR M&M Ready Mix concrete
STREET 1933 E. 16th Street
CITY/STATE/2IP Greeley, CO 80631
Gv~ moao ~~ r~z y~c,
cc: Carl B.
