• (Page 2) • <br /> MINE ID # OR PROSPECTING ID # M-1977-451 <br /> INSPECTION DATE 1216/00 INSPECTOR'S INITIALS WHE <br /> OBSERVATIONS <br /> This inspection occurred in response to the operator's request for a $9,418 reduction in financial <br /> warranty, from the existing $9,718 to $300, for completion of reclamation backfilling, grading and <br /> topsoil replacement at the Ike No. 1 Mine. The Ike No. 1 Mine permit is approved for 13.3 acres <br /> affected area and is located within the DOE lease block C-SR-11 . DMG records indicate that a <br /> total of $17,718 financial warranty is required for the operation. The $17,718 f warranty <br /> is in the form of an $8,000 bond nd U2475575, and a $9,718 bond with DMG, <br /> corporate surety ESD-7787405. <br /> Permit History The original 110 permit was issued to W.S. Dawson for 2 acres affected area, <br /> located in portions of the NW '/s of the SE '/4 of Section 8, T43N, R19W, NMPM, San Miguel <br /> County. On 1/24/85, DMG approved SO-01 , for Cotter Corporation as successor operator. On <br /> 10/18/90, DMG approved CN-01, converting the 110 permit to a 112 permit. The application form <br /> for CN-01 indicates that 11 .3 acres were added to the permit, located in portions of the W % of <br /> Section 17 and E % of Section 18, T43N, R19W, NMPM, San Miguel County. Therefore, under <br /> CN-01 the permitted acreage was increased to 13.3 acres. On 1/9/00, DMG approved TR-01 , <br /> approving the $8,000 bond held by DOE for Cotter operations within DOE lease block C-SR-11 , <br /> as financial warranty. <br /> Portal Area The mine identification sign was posted at the mine entrance. The portal decline had <br /> been backfilled and graded with waste rock materials. Available topsoil had been replaced. The <br /> portion of the waste rock stockpile, recently disturbed to provide backfill material for the portal, <br /> had been graded to approximate 3H:1 V slope. Vegetative cover for the remaining waste rock <br /> stockpile area, not recently disturbed, was diverse and well established. The recently disturbed <br /> areas, where vegetative cover has not had sufficient time to establish, was visually estimated at <br /> 0.5 acres at the portal area. <br /> The venthole had been sealed with foam and backfilled. Backfill material had been mounded over <br /> the closed venthole, per DOE requirements. The access road to the venthole had been obstructed <br /> with tree trunks. The recently disturbed areas, where vegetative cover has not had sufficient time <br /> to establish, was visually estimated at 0.25 acres. <br /> The water tank had been removed and the area ripped and rough graded. The recently disturbed <br /> areas, where vegetative cover has not had sufficient time to establish, was visually estimated at <br /> 0.25 acres. <br /> Backfilled and graded areas appeared stable, evidence of settling, slumping or excessive erosion <br /> was not observed. Available topsoil and native earthen materials had been replaced. Mining <br /> related structures had been removed. Remaining reclamation appeared to be limited to <br /> revegetation of approximately 1 acre area. <br />
