The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE ~1. The permit ID sign is out-of-date, and the permit boundaries are not marked, though 5/15/05 these items are required to be kept current and maintained, per Rule 3.1.12. PB or PV: PB TOPIC(S): SM CORRECTIVE ACTION: The operator must update the sign, and accurately locate and _, _, mark the permit boundaries, as discussed in this report. Evidence of completing this action must be received by this office by the date shown at right. ~2. The transfer of the permit was requested in 1993 but was never approved, though if the 5/15/05 permit is to be operated commercially it must be transferred. PB or PV: PB TOPIC(S): AR CORRECTIVE ACTION: The operator must submit a permit transfer request (on form _, _, provided to operator), and obtain the transfer approval by the date shown at right. ~3.
