The affect ed area boundaries were not marked by 07/31/98 <br />monuments or other markers as required by Rule PB or PV: PB <br />3.1.12(2) TOPIC(S): SM <br />Corrective actions are to: <br />1) Mark the boundaries of the affected <br /> area with markers that are clearly <br /> visible and adequate to delineate <br /> such boundaries. <br />2) Submit a photograph to our office <br /> which clearly shows that the affected <br /> area boundary markers have been <br /> installed. <br />2. The permit application on file with the Divison is 7/31/98 <br />deficient in several areas and the operator needs to PB or PV: PB <br />submit a t echnical revision to the permit to address TOPIC(S): RS, <br />the follow ing: TS, HB, MP <br />1) Submit map with information <br /> sufficient to determine the location <br /> of the affected areas in the field <br /> and to be able to mark the affected <br /> area boundaries in the field. <br />2) Clarify the topsoil handling <br /> procedures for this site including <br /> salvage procedures, locate storage <br /> areas on the above map and identify <br /> temporary seed mix to be used to <br /> protect topsoil while it is being <br /> stored. <br />3) Identify the expected depth to which <br /> mining will occur and modify the <br /> reclamation plan to clarify what the <br /> post-mining topography will be, <br /> including the location of the Sand <br /> Creek channel and the location of any <br /> ponds that may remain once mining is <br /> completed. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />
