(Page 3) <br />MINE ID Jl OR PROSPECTING ID rY M-1976-032 <br />INSPECTION DATE 5-25-2000 INSPECTOR'S INITIALS SSS <br />To satisfy the outstanding corrective action requirements, Telluride Gravel must submit documentation indicating that the <br />USFS has granted right of entry to the site. Additionally, a technical revision must be submitted that indicates the proposed <br />plans for reclamation of this area. The permit maps must also be revised to illustrate these plans. <br />The Board Order to cease and desist from operations on USFS lands within the permit boundary until right of entry to those <br />lands is secured, appears to have been disregarded. The requirement to submit, and secure approval of, a technical revision <br />addressing proposed operations for the USFS property, prior to conducting that work, also appears to have been <br />disregarded. With unpermitted work already taking place on the USFS property, the requirement for the Board ordered <br />documentation must now be submitted. Failure to provide the right of entry and the technical revision at this time may <br />result in an enforcement action for failure to comply with a Board Order. <br />The reclamation cost estimate will be recalculated in the next few weeks. The costs will be based on current site <br />conditions, and requirements of the approved reclamation plan.
