PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE Several deviations from the approved permit specifications were noted during this 5-15-2001 inspection and are noted in the foregoing report. The revision must be received by the correction date and must address specific deviations from the approved permit for each of the items detailed in the body of this report such as the revised configuration and dimensions of the highwalls, the two (or morel bench mining method, the increased maximum disturbances, the lack of contemporary reclamation, the new and unauthorized asphalt plant, the lack of (or re-disturbance ofl reclamation on the west parcel, the revised topsoil/overburden stockpile location, and the increase in reclamation costs due to these changed conditions. 2. Several large diesel fuel tanks were noted throughout the site that were not placed 4-30-2001 within adequate spill containment structures. PB or PV: PB TOPICISI: AT CORRECTIVE ACTION: HS , _, _ Either remove all of these tanks from the permit area and submit photographic documentation of compliance with this corrective action, or submit a revision application addressing incorporation of specifications for construction and reclamation of adequate spill containment structures for these tanks. 3. The six nesting boxes that were to be installed in 1998, on the north side of the west 5-15-2001 pond, have not yet been constructed. The topsoil/overburden berm on the south side of the Amendment No. 1 parcel does 5-15-2001 not support an adequate vegetative cover.
