(Page 2) <br /> <br />MINE ID # OR PROSPECTING ID # M-78-014 <br />INSPECTION DATE 05/27/99 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a routine inspection of the site <br />1) Signs and boundary markers are in place as required by the rules <br />2) The site was too muddy to inspect completely, or to use the measuring wheel to gauge the <br />size of the stockpiles. <br />3) Most of the site (with the exception of the northeast corner, approximately 5 acres in <br />size) appears to be disturbed at this time, with probably enough topsoil present to reclaim <br />the site. It is currently being mined. <br />4) Setbacks from the power lines were being observed as required in the permit. <br />5) Stormwater runoff was pooling in one of the excavation cuts; no drainage appears to be <br />escaping the site. <br />6) The diesel fuel tank in place on the site should have a berm or catch basin below it, in <br />order to prevent spills to either surface or groundwater. (P81) <br />A second inspection will be set up within a month, to confirm that the financial warranty <br />currently posted is adequate and to measure the size of the disturbance and topsoil <br />stockpiles. <br />No other problems were noted at this time. <br />I & E Contact Address cc: Carl B.
