Suk~mit the required financial warranty associated <br />with the approval of this conversion to this office <br />within 30 days. This amount will be $48,050.00 if <br />thE~ appropriate well permit and water rights for <br />evaF~orative loss after reclamation are also <br />submitted or; $81,290.00 if the well permit and <br />evapox'a five loss information is not submitted with <br />the financial warranty. <br />The operator failed to submit verification that they 12/31(98 <br />have obtained a well permit and substitute supply plan PB or PV: PB <br />from the State Engineer's Office to conduct a mining TOPIC(S): HB, <br />operation that exposes groundwater.
