T <br />Prior to our responses below, we note that in our discussions with OMLR staff today, it may <br />have been assumed by some of the staff that the observations of July 14, 1994 related to the <br />main underdrain or to an underdrain that was anticipated to carry a measurable amount of flow. <br />As was recognized in the inspector's report and as is further described later in this report, this <br />drain was a "secondary finger drain" in which "some dampness" was observed. Further, we <br />underscore that prior to the inspector's amval on the site, placement of material that was less <br />suitable had been halted by Golder and Bateman and removal and replacement had been <br />scheduled. <br />Critical Components Requiring_Frequent Field Ouality Assurance and Quality Control (l/31: <br />Addressing the OMLR questions in a different order than listed, CC&V believes the following <br />are the critical components that require frequent QA/QC inspections in the context of the Mined <br />Land Reclamation Board Permit M-80-244: <br />1. Leach facility foundation including toe berm foundation preparation and <br />placement of structural fill. <br />2. Underdrain installation (pipes, pipe bedding material, drain rock, structural fill <br />cover. geotex[ile and VLDPE). <br />3. Leak detection trenches and sumps (configuration, placement of liner, placement <br />of drain pipe and gravel material). <br />4. Toe fill berm (material/placement/compaction). <br />5. Soil liner fill (material/placement/compaction). <br />6. Geomembrane installation (oversight of installation and testing). <br />7. Geotextile installation (surface preparation, placement, cover). <br />8. Placement of leak collection pipes, access tubes, piezometer, and gravel material. <br />9. Placement of pump caisson bearing surfaces and caissons and placement of <br />surrounding fill. <br />10. Liner cover (gravel) installation (material/placement). <br />Frequency of Field Quality Assurance for Critical Components (#I): <br />The frequency of QA/QC visual, in-field, inspection (as contrasted with laboratory testing) will <br />vary slightly between some of the critical components.
