• (Page 2A~ <br />1 MINE ID # OR PROSPECTING ID # M-87-178 <br />INSPECTION DATE_Q~[3Q[28_ INSPECTOR'S INITIALS RCO <br />This inspection was performed by the Division as part of its monitoring of 112 construction <br />material permits. The operator was contacted about the inspection and the operator and his <br />representative, both named on page one of this report, were present throughout the <br />inspection. <br />This inspection was to monitor the operation within the existing 112 permit area and to <br />perform the pre-operation inspection of the area to be added in the recent amendment <br />application. This report will describe these two parts of the inspection separately, <br />~ Mons or+na +nsn ion o xis ~ng~erm+t a ea <br />The permit ID sign was posted on the entrance gate. The gate is on a paved access road, <br />which all mine traffic has utilized in the past and is still utilized presently. (This road <br />will be used for all future operations, after amendment AM-O1 is approved.) The permit <br />boundary is adequately marked, on the south and east by the fencelines which delineate the <br />permit and the surrounding farm fields, and on the west and north by the topographic break <br />(the bluff) between the river terrace and the bottomland. <br />There was no disturbance to any areas off site, by mining, sloughing or sedimentation. There <br />are two "wild" gullies, which are specifically being avoided, for their sensitivity related <br />to runoff and wildlife. (These gullies were referenced in the CDOW letter in the AM-O1 <br />application.) <br />The git operation was active on tyre day of the inspection. Truck traffic was entering and <br />exiting the pit, hauling material from stockpiles in the floor of the pit. There was no <br />accumulation of mining debris, contaminated soil or water, or inadequately stored fluids. <br />The pit has advanced nearly to its eastern and southern limits. The slopes there have been <br />graded to 3:1, though not topsoiled or revegetated because they are interim slopes. These <br />will eventually be mined further to the east and south after AM-O1 approval, but are <br />adequately stabilized for now. <br />The pit floor is about 30 feet below the unmined areas surrounding it. It is fairly level, <br />and is at about the final grade for reclamation. There were no areas of obvious impoundment <br />of water, except for the runoff/sediment ponds at the west end. <br />The far west end is where the topsoil and overburden storage are. They are out of the way <br />of disturbance, in stable configurations and appear to be of sufficient volumes for current <br />reclamation needs. Vegetation covered the piles, but the type of grass was not able to be <br />field identified. <br />There were no problems noted during the monitoring inspection of the existing permit area <br />2 Pre-operation +nspect+on of areas to be added ~n amend_ment aon~+cat~on AM_Oi <br />The fields east, south and west of the present permit area will be added in this amendment. <br />These were viewed, and exhibit no present mining disturbance. The north edge of the amendmed <br />area, called Area 3 in the application, is on the river terrace, but is immediately above the <br />
