Drainage report and plan (signed by a drainage engineer) that <br /> includes (1) map(s) showing the location of the existing and proposed mine <br /> facilities, the locations of streams, rivers, reservoirs, springs, or other surface <br /> water features within the permit area., (2) analysis of on and off-site drainage <br /> impacts, (3) a drainage plan showing the surface water management system <br /> including the routes of water directed off the proposal area which could affect <br /> receiving waters outside the proposal area, (4) wells addressed in state <br /> discharge permits or storm-water runoff management plans, and (5) copies of <br /> applicable water quality discharge permits issued or applications submitted for <br /> the proposed development or operations. <br /> 5.7 Drawing - See the requirements of Section 6 <br /> 5.8 Fee - Application fee based upon area to be included in PUD <br /> 0 to10 acres $250.00 <br /> 10.1 to 50 acres $300.00 <br /> 50.1 to 99 acres $500.00 <br /> 100 + acres $1500.00 <br /> 5.9 Impact Study - including the following information <br /> 5.9.1 Air Quality - Information. A copy of applicable air quality permits, including <br /> pertinent permit applications for authorizations applied for but not yet <br /> received that are applicable to the proposed activities. If permits or <br /> applications are not yet available and new or revised permits are required <br /> for the proposed activities, the submittal shall include the time-schedule <br /> for obtaining the requisite authorizations. <br /> The application shall include also (1) a map showing the location of <br /> the facilities and which depicts the general location of existing and <br /> proposed activities that are considered areal or point air emission sources <br /> for which Air Pollution Emission Notices are required from the Colorado <br /> Department of Public Health and Environment CDPHE-Air Pollution <br /> Control Division, or successor agencies , (2) how potentials for off-site <br /> impacts from air quality are managed or controlled to protect public health, <br /> and (3) the location of air monitoring station(s) used to provide monitoring <br /> data to the State or federal regulatory agencies on the map provided to <br /> Teller County. These showings may be satisfied by the submission of <br /> applications for, or permits drafted or issued for, the proposed new <br /> releases of air emissions, when subject to regulation, from the proposed <br /> mining operations. <br /> 5.9.2 External Effects - Information. Analysis of potential impacts including a <br /> description of off-site impacts generated as a result of the operation, <br /> including light or glare, noise, vibration, blowing or flowing material from <br /> the site, trucks or haul roads, as well as the proposed methods to mitigate <br /> these impacts. <br /> 5.9.3 Hazardous Substances - Information.
