<br />2t1 <br />20 INar. '96 DMG (H. Humphries) ltr to Fox Engineering (R. Fox) denies COM, lnc's request for <br />a deadline extension to April 12 for completion of installation and test of the spray <br />evaporation system noted above. <br />1 A,pr. '96 COME (R. Mason) visual inspection of tailings pond area from offsite indicated that <br />spray system installation and test was not complete, tact that work was is progress. <br />Inspection of the Hazel A. indicated that the coffer dam and putnpback system <br />were not functioning as designed; water was overflowing the top of the coffer dam <br />and seeping into soil under the alit access cniveR and presumably outward towed <br />Cash gulch. Flow rates were [ow, tto more than ll2 to 1 plat per minute. DMG and <br />CDH WQCD personnel were contacted. <br />2 Apr. '96 DMG (A. Sorenson) I,aspectioa Report identities 1 P$; inadvertent disconnection of <br />power to the pump-back system. Required to establish and maintain a log at the Hazel <br />A portal. Required to operate the spray evaporation system daily as long as the <br />water level in the pond is above the level of the pump intal[e. Required to <br />maintain a log which docamems the tailing pond water level prior to and following <br />each pumping episode, See Observations section for further requirements. <br />3 Apr. '96 COME (R. Mason and !. Nilson) returned to the Hazel A with video camera and 33 mm <br />stiD camera d7 photograph situation at the alit. DMG was advised of this action. <br />4 Apr. '96 COME (R. Mason) contacts DMG (A. Sorenson) and is advised that failure of puttrpback ` <br />system was due to inadvertent unplugging of sump pump. We discuss corrective actions <br />required, including upgrading of atilt electrical system to appropriate standards. Sorensen <br />indicates that sump pumping has been resumed and DMG will follow up on corrective <br />actions. No fitrtttu action taken by COME. <br />11 Agr. '96 COM, lnc. (R. Foz) submits, via FAX, a system design f'or a land application system for <br />Hazel A water, to be used as backup far the pond evaporation rystem. DMG (A. <br />Sorenson} responds oa the same day, stipulating that neither ponding nor runoff 5vm <br />the application area is permitted, and that nay such may be a violation of the Colorado <br />Water Quality Coarrol Act. Response notes further that the application is subject to <br />inspxtion by DMG without notice, and that a water quality analysis of the Hazel A water <br />is teyuired. <br />12 Apr. '96 DMG (A. Sorenson) !tr to COM, Inc. (R. Fanyo) indicates that water quality data <br />submitted show contamination beyond allowable limits for drinking water, human <br />health, and aquatic life. Significant contaminants include mercury and selenium. <br />Despite this, the Division approves the land application system, subject to the provisos <br />noted above, and with the further stipulation that the spray evaporation system is to be <br />used av the primary mecttauistn for dispos;ng of process water (i.e, Haul A and tailings ~ <br />pond water). <br /> <br />
