A site inspection of the Phillips (Nolan) Stone Quarry was conducted in response to the operator's request for a permit <br />amendment to increase the acreage of permitted and affected land from 3.0 to 9,9 acres. The request is under review by <br />the Division via AM-1. Present during the inspection was Robert J. Phillips. <br />2. The Nolan Quarry is approximately 2.5 miles north of Lyons, Colorado. The site is currently divided into two (2} <br />separate quarries for a total permit area of 3.0 acres. The upper quarry is approximately 1.1 acres and the lower quarry is <br />approximately 1.9 acres. The operator has posted a financial warranty of $1,015. The post-mining land use is home site <br />development. <br />3. A permit I.D, sign was posted at the entrance to the lower quarry. Permit boundary markers were also noted and all <br />disturbances were found to be within the permit boundary. <br />4. The operator has proposed, via AM-1, to maintain a maximum disturbance at any one time of 3.0 acres. The quarry <br />highwalls will be a maximum of 15' during mining and will be reduced or backfilled to a maximum height of 6' during final <br />reclamation. There will be a maximum of 1,000 tons (787 cubic yards) of overburden to bac~ll and grade to reclamation <br />contours during final reclamation. The operator will also re-vegetate a disturbed area of 3.0 acres. The Division has <br />calculated a total reclamation cost of $1,400 for this planned disturbance, See attached reclamation cost estimate. <br />5. At the time of inspection, this inspector estimated 150 linear feet of highwall @ 12' for the upper quarry, and 150 linear <br />feet of highwall @ 8-10' for the lower quarry. <br />6. Digital photographs were taken of the upper and lower quarries and are attached to this report. <br />7. The Annual Report & Annual Fee for the previous year (2004) are due on the permit anniversary date, July 30. <br />8.
