• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-77-231 <br />INSPECTION DATE 3/4/98 INSPECTOR'S INITIALS NA <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The Boundaries of the affected area were not marked by 5/29/98 <br />monuments or other markers that are clearly visible and PB or PV: PB <br />adequate to delineate such boundaries as required by TOPIC(S): SM <br />Rule 3.1.12(2) <br />Corrective actions are to: <br />1) Clearly mark the boundaries of the <br />affected area so as to comply with <br />rule 3.1.12(2). <br />2) Submit a photograph to our office <br />which clearly shows that the boundary <br />markers were installed. <br />2. The permit application does not contain maps which 5/29/98 <br />adequately depict the location of the affected area, do PB or PV: PB <br />not clearly show the mining plan, and do not clearly Topic(s): AR, <br />show the reclamation plan. Specifically, the maps MP, PB, TS <br />do not identify pit locations, topsoil stockpile <br />locations, product pile locations, processing areas <br />etc. <br />Corrective actions are to <br />1) Submit a new Exhibit E map(s) in <br />accordance with Rule 6.3.5 which <br />clearly shows the affected area <br />boundaries, all components of the <br />mining plan (ie: pit locations, <br />product stockpile locations, topsoil <br />stockpile locations, etc.), and all <br />components of the reclamation plan <br />(ie: gradients of regraded slopes, <br />thickness of overburden replaced, <br />thickness of topsoil replaced, etc.). <br />
