The following problems (PB) and/or possible violations (PV) (and suggested
corrective actions) were identified during this inspection. The problems
should be corrected by the dates given, or they will become possible
violations. The possible violations should be corrected by the dates given
to reduce their severity when considered by the Mined Land Reclamation
Board. The inspector noted on the previous page should be notified of all
corrective actions taken.

1. Revegetation is not being performed in a timely 12/1/98
fashion, though it is required to be, by the permit
and the performance standards in Rule 3.1.5. PB or PV:PB

The operator must commence the revegetation portions
of the approved reclamation plan. If there are to be
no changes to the plan, the operator must follow the
existing plan. If the operator wishes to submit a
proposed change to the plan, that revised plan must be
followed, upon its approval.
In either case, the operator must begin revegetation
activities, according to the plan which is approved at
the time of the corrective action date, and provide
written evidence of such activity, by the date shown
at right. --->
2. Topsoil is not being salvaged or stockpiled on the 03/31/98
site, in amounts sufficient to perform the approved
reclamation plan. PB or PV:PB

The operator must provide a statement, as described in
this report, and according to Rule 3.1.5(9), detailing
the source, volumes, areas of use, etc., for the
topsoil now required in the plan.
