(Page 2) <br />MINE :CD # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 09/14/99 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during an inspection at the site made at the request of <br />the Operator. The site will be going into Temporary Cessation of Operations, and a final <br />inspection prior to that was believed to be necessary. <br />1) Gates and fencing for the site, preventing access from animals and unauthorized members <br />of the public, as well as the required signs, are locked and in place. <br />2) A layer of fill has been placed and compacted on the downstream face of the dam, <br />minimizing erosion to the greatest possible extent at this time. However, the silt fence <br />below the downstream face has collapsed again, and the loose rocks on the face of the dam <br />do not appear to be stable. Plans for seeding are in the works, and the possibility of <br />better erosion control than the silt fence has been discussed. Changes to erosion control <br />may require a technical revision if they involve significant structural changes around or <br />below the toe of the dam. <br />3) The interior of the mill has been cleaned out, with all of the clarifier tanks emptied of <br />tailings, all of the sumps and sifters emptied of water and tailings, and all of the <br />ec,~u ipment fixed and ready to be operated. A new ball-table was never installed, but the <br />area where it would be stored is still clear in case plans change. <br />4) The stormwater controls on the east of the site, including berms and sediment basins, have <br />been installed and appear to be adequate for draining the ore storage area. <br />5) On-site maintenance will be done weekly by Max Yeager. <br />6) A yardstick or tape with the height marked in feet needs to be installed or affixed in the <br />pond, so that the height from the top of the dam can be checked easily, allowing the <br />Operator to initiate pumping operations from depth readings, if necessary. DMG will <br />discuss triggers for pumping operations with the Operator in the review of the Temporary <br />Cessation application.. (Y81) <br />No other problems were noted.
