<br />INSPECTION REPORT <br />DATE: November 15-17, 1994 <br />MINE: Roadside Portals <br />PERMIT NO.: C-81-041 <br />COMPANY: Powderhorn Coal Company <br />P.O. Box 1430 <br />Palisade, CO 81526 <br />Individuals Present During the Inspection: <br />Larry Reschke, Powderhorn Coal Company <br />Erica Crosby, Division of Minerals and Geology <br />Russ Porter, Office of Surface Mining <br />This was a random sample oversight inspection of the above <br />referenced permit. The weather during the inspection was cool <br />with sun and clouds and snow on the last day. The ground was <br />dry before the snow. <br />RECORDS REVIEW <br />The current permit was issued on January 3, 1993 and expires <br />on January 3, 1998. The mine which used to be known as the <br />Roadside and Cameo mines has had a name change and is now <br />called the Roadside north and south portals or Roadside <br />Portals. The mine is an underground operation which is <br />located east of the city of Grand Junction, Colorado along <br />interstate 70 and the Colorado River. The portals are located <br />on both sides of the interstate and the river. <br />Liability insurance is provided by policy ~CGO G1 658779-5 <br />which became effective on October 1, 1994 and expires on <br />October 1, 1995. The general aggregate amount of coverage is <br />$2,000,000.00. The policy provides coverage for all of <br />Powderhorn Coal Company (PCC) operations in Colorado. The <br />notification language, for cancellation of the coverage, <br />stated that the insurance company "will endeavor to" notify of <br />a cancellation. This language has been determined to be <br />unacceptable. The company was in the process of getting the <br />cancellation language changed.
