<br />MINE ID ~ OR PROSPECTING ID $ <br />INSPECTION DATE 2 9 96 <br />(Page 2) <br />M-94-117 <br />~, <br />INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />A two man crew from Colorado Lining Company was on-site at the time of this inspection <br />finalizing repairs to the tailing pond liner. The crew from Colorado Lining Company was <br />being assisted by a three man crew provided by COM, Inc. The equipment and methods being <br />employed by Colorado Lining Company to complete the liner repairs were extensively observed <br />over a 4 hour period, and were found to be in accordance with the high standards expected of <br />the eiynthetic liner industry in general. Actual vacuum testing of the repairs was not <br />observed, however, the vacuum box apparatus was inspected and found to be acceptable. In <br />certain locations around the periphery of the pond, the presence of ice made it difficult to <br />expose the liner for inspection. It was determined that breaking the ice to remove it would <br />result in additional damage to the liner, and that it would best to leave the ice in place. <br />If any liner defects are exposed when the ice melts, they will have to be repaired. <br />The Hazel-A adit was inspected. Backfill around the corrugated metal pipe that was used ae <br />a portal closure ie beginning to separate from the back of the adit at the portal. Shis <br />situation will have to be monitored and redressed if portal security is threatened. <br />The pump-back system behind the coffer dam in the Hazel-A remains frozen, There was still <br />aubst:antial freeboard behind the coffer dam at the time of this inspection. It ie clear that <br />a tank heater will need to be installed to assure the performance of the pump-back system. <br />See 1'B ,}1 on the last page of this report. <br />Water was impounded at a level approximately half way up the Hazel-A bulkhead. The Hazel-A <br />pumping schedule was interrupted to facilitate tailing pond liner repairs. Water from behind <br />the Ffazel-A bulkhead will be pumped up to the pond as soon as the liner repairs are complete. <br />Only very minor leakage through the bulkhead was observed. The plastic liner on the upstream <br />aide of the Hazel-A bulkhead is still not adequately affixed to the rib. See PB f2 on the <br />last page of this report. <br />The operator reported on their intention to seal the borehole that enters the Hazel-A just <br />inby of the portal.
