<br />I D. <br />JUL 1~5 21 "III IIIIIIIIIIIIIIII <br />Cripple Creek & Victor Gold Mining Company <br />FINAL DRAFT <br />SPILL REPORT <br />Date of report: 7/12/95 cc: Jim Stevens <br />Tom Schreiner <br />FAXed to: 303-832-R106 (w/o utap) Berhan Keffelew <br />Brief summary: On July 6, 1995 a tnaxintum of 1000 gallons of weak neutralization <br />solution was released onto the ground by Pad 3 when a butt weld on a solution pipe <br />partially failed causing a portion of the solution to spray beyond the liner. <br />Date of spill: 7/6/95 <br />Time start: 1815 est. <br />Time stop: 1830 <br />Cause: partially failed 4" HDPE pipe butt weld <br />Volume: < 1000 gal. est. <br />NaCN concentration: <0.025 Ibs/ton of water (<0.1 lbs. NaCN) <br />Emergency measures to neutralize and contain: <br />- Opened valves to relieve pressure to stop overspray. <br />- ]3ei•med dirt road beside pad to contain spill. <br />- Treated with hypochlrn•ite (1S lbs. liquid and 5 lbs. solid). <br />Additional corrective actions: <br />- Disconnected pipe with failed weld until the other welds could be checked and the <br />failed weld could be ropairY;d. <br />- Sampled area that came in contact with the spill to determine if additional <br />remediation is necessary (completed 7/10/95 by Scott Wanstedt and Jim Stevens). <br />- Analyu solid sample via Meteoric W atcr Mobility Test (per Posey) for CN WAD, <br />Zn, anct Hg (per Keffelew) (results pending). <br />- Sample solution being applied to Pacl 3 to characterize solution released (results <br />pending). <br />
