Terror Creek Coal Company <br />Terror Creek Loadout <br />1840 Hwy. 133 <br />Paorua, CO 81428 <br />August 6, 1996 <br />Routine Complete Inspection <br />Participants: <br />Mike Boulay/Dan Matthews, DMG <br />Kelly Fritzler, Terror Ck. <br />Bryan Bennett/Henry Austin, OSM <br />We participated in a joint, complete inspection of the Terror Creek Loadout with the above <br />participants representing the Colorado Division of Minerals and Geology (DMG); Terror Creek <br />Coal Company; and the Office of Surface Mining. The weather was clear and hot and ground <br />conditions were dry throughout the site. Mr. Bennett participated in his capacity as collateral duty <br />safety officer of the Denver OSM office and conducted a safety evaluation of both the inspection <br />and government vehicle usage. <br />Mr. Boulay began the inspection by identifying us all to on site company personnel and orienting <br />us to the site by reviewing appropriate permit maps depicting the site facilities, permit boundaries, <br />and surface water runoffcontrol structures. We began the field evaluation by walking and <br />observing all disturbed areas of the Loadout facility which encompass approximately 13 acres. We <br />walked east to west and made general observations of the topsoil piles; diversion ditches leading <br />to the site's one sediment pond; signs and markers which are required; the 3 approved alternative <br />sediment control areas(ASCA); and finally the truck parking/garage area. <br />The sediment pond was completely dry and has never discharged according to inspection <br />participants and quarterly discharge monitoring reports. The pond spillways and embankment <br />were stable and functional. Mr. Boulay has recently directed the permittee to conduct weed <br />control on the topsoil piles as necessary; reseed the coarse textured (rocky) topsoil stockpile; <br />and replace siltation fabric as necessary at the ASCA's. Mr. Boulay instructed the permittee to <br />replace additional siltation fabric and this was completed during the inspection. Topsoil stockpiles <br />are stable with no signs of excessive erosion. <br />The main (northern) diversion ditch to the sediment pond had been recently maintained and is in <br />good operating condition. The permittee has also recently resurfaced the working areas around <br />the garage and truck parking areas. These areas are stable and well maintained and surface water <br />runoff is being controlled as approved in the permit. This completed the field portion of our <br />inspection. <br />
