i
(Page 2)

MINE ID t OR PROSPECTING ID ~ M-77-247
INSPECTION DATE July 6, 1995 INSPECTOR'S INITIALS TAS
OBSERVATIONS
1. The Domtar Quarry was inspected to evaluate the progress of stormwater control
installation. Site conditions were dry and hot.
2. The operator has repaired approximately 540 linear feet of collection ditch from the
sediment pond to the culvert at the entrance to the mine shop. A 2-foot deep trapezoidal
channel has been formed and is covered with "Pyramat," a synthetic erosion control fabric.
The fabric has been secured in place and will be covered with a thin layer of soil.
Following the soil application, the channel will be reseeded with the reclamation seed mix.
3. The operator has done an excellent job of repairing the inlet to the collection pond.
The inlet ie now stable and is capable of handling future runoff at the site. An emergency
spillway will be installed in the collection pond during the 1995 field season.
4. The extent of repairs made thus far to the collection ditch comprise approximately 2/3
of the necessary channel repairs. Remaining repairs should be completed by August 31, 1995.
5. Runoff will also need to be controlled at the large, flat area where the crusher is
located. This area receives runoff from several upgradient watersheds. There should be a
series of small collection ditches installed to direct the incoming runoff toward the main
collection ditch leading to the sedimentation pond.
6. Problems with runoff and sediment control were first noted by the Division during a
November 2, 1994 inspection. Repairs were then requested. The Division fully expects that
a complete system of etormwater controls will be installed at the Domtar Quarry no later than
September 30, 1995. Enforcement action may become necessary if the requested repairs are not
completed by that date.
I & E Contact Address
NAME Mr. Ren Rlco
OPERATOR Azurite, Inc.
STREET 10001 CR 12
CITY/STATE/ZIP P.O.
