OR PROSPECTING ID X M~0-146 <br />INSPECTION DATE 519196 INSPECTOR'S INITIALS WHE <br />The following problems IPBI andlor possible violations (PV 1 land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Perimeter markers were not observed for the north boundary. Rule 3.1.1212) states that the boundaries of the affected area will <br />be marked by monuments or other markers thei are clearly visible and adequate to delineate such boundaries. Rule 3.1.12121161 <br />defines "affected area" as the area proposed to be disturbed by mining operations for which a financial warranty and performance <br />warranty have been posted. <br />CORRECTIVE ACTION: Post and maintain affected area perimeter markers as required by Rule 3.1,1212). <br />6/14196 <br />PB or PV PB <br />TOPICISI SM <br />2. A natural gas pipeline and pipeline access road appeared to be located within 200' of the affected area but were not identified <br />on the Pre-mining and Mining Plan Maps, as required by Rule 6.4.316) and Igl. Topsoil and overburden stockpiles were located <br />in close proximity to the pipeline location but did not appear to overlay the pipeline. Mining activities did not appear to have <br />adversely affected the stability of the pipeline or access road; however, the permit does not contain a compensation for damage <br />agreement or appropriate engineering evaluation that demonstrates that the pipeline will not be damaged by mining activities, <br />as required by Rule 6.4.19. <br />CORRECTIVE ACTION: Submit a revised Mining Plan Map, which identifies the pipeline and pipeline access road, in accordance <br />with Rules 6.4.3(61 and Igl.
