The inspector noted on the previous page should be notified of all corrective actions taken. <br />j?ROBLEMS/POSSIBLE VIOLATIONS <br />CORRECTIVE ACTIONS <br />CORRECTION <br />The operator had dlsposcd of sediment and run heavy equi ent on the padjacent landowner's 07 / 12 /06 <br />ar%a~of a~~proon~y~ ley)O The area aal~eg~excav~ible nviolaG~rwiF~ ~Sro34-32 ~5 116 4Xrj a~ <br />Rule 3.1.5(3) for failure bo protect areas outside the affected lands from slides or other damage. <br />P8 or PV: ~(._ <br />TOPIC(S): ~D. <br />Cortective action: On or before the July 12, 2006 Board Meeting, the operator must complete the - - - <br />folknving actions: 1) Submit a boundary survey that dearly establishes the properly boundary <br />between SW TKO's and Tony Finley's propertles, 2) Conduct a field survey of the property boundary <br />between those properties and establish adequate permit boundary markers in accordance with the <br />properly boundary survey, 3) Remove all existing sediment to the extent practical from Mr. Finley's <br />property, 4) Re-vegetate all disturbed areas in accordance withthe re-vegetation plan for the Lupton <br />Lakes Resource, 5) Backfill and grade and stabilize any disturbances or excavations on Mr. Finley's <br />property created by excavators, loaders, or other heavy equipment irnolved in the drainage channel <br />stabilization project, and 6) Submit photographs or other evidence verifying completion of all these <br />tasks. <br />2. There was significant gully erosion in the northeast comer/embankment of the drainage 07 / ~ /06 <br />stabilization channel, that makes the foundation of the drainage ccfihaannel embankment unstable and <br />prone to further erosion during future storm everrtr. <br />PB or PV: ~ <br />TOPIC(S): ~ <br />Corrective Actions; Complete stabilization of the erosion within 30 days. Submit a plan and - - <br />completion report to the Division following wmpletion of the stabilization work which describes the <br />stabilization plans and work completed. The plan and completion report must be signed and stamped <br />by a registered, P.E. with experience in drainage stabilization, The plan and completion report must <br />also include photographs or other evidence rerifying oompleton of this work. <br />
