4 <br />Narrative Inspection Report <br />Date: September 10, 1992 <br />Mine: Terror Creek Loadout <br />Permit No: C-83-059 <br />Company: Terror Creek Company <br />1840 Highway 133 <br />Paonia, CO 81428 <br />This was a complete random sample inspection. <br />Those present during the inspection were: <br />Dave Berry, MLRD <br />Christine Johnston, MLRD <br />Stephen Rathbun, OSM <br />Weather: Sunny, cool. Ground conditions were dry. <br />RECORDS REVIEW <br />Certificate of insurance: A policy with appropriate amounts is <br />in place. It expires on 7-21-93. The notification statement <br />needs to be revised to remove the phrase will endeavor". Ms. <br />Johnston has notified the Albuquerque Field Office that she <br />will require TCC to revise the policy in the very near future. <br />Bond: A certificate of deposit for $96,550.95 is in place. <br />NPDES permit: COG-850028 issued on 7-1-92 expires 6-30-97. <br />Second quarter '92 was reviewed with no discharges noted. <br />Water monitoring: No groundwater monitoring required by MLRD <br />permit. Surface water monitoring consists of compliance with <br />NPDES requirements. <br />Air quality monitoring: 82DL343 (3F) - Coal Loadout and <br />Stockpiles; (2Ff - Haul Road; issued 9-26-88. Care needs to be <br />taken to ensure that the Mag Chloride requirements for the <br />haul road are complied with in the Spring/Fall. Also, that the <br />spray bars are utilized during use of all crusher and conveyor <br />equipment. <br />SPCC plan: issued 3-4-85 with no problems noted. <br />Pond inspections: Second quarter '92 submitted 6-24-92 with no <br />problems noted. <br />
