(Page 3) <br />MINE ID # OR PROSPECTING ID # M-1978-315 <br />INSPECTION DATE 6/9/03 <br />INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />According to the annual reports submitted by Routt County, the <br />site was last mined in 1993 where 0.5 acre was disturbed. Rule PB or PV:PB <br />3.1.3 of the Construction Materials Rules and Regulations TOPIC(S):RS <br />requires reclamation to be completed within 5 years. August 8, 2003 <br />or November 28, <br />The operator must demonstrate that mining has taken place within 2003 <br />the last 180-days, apply for Temporary Cessation status by <br />August 8, 2003 or reclaim the site by November 28, 2003. In <br />addition, documentation of the reclamation must be submitted to <br />the Division in the form of photographs or receipts. <br />
