(Page 2) <br />MINE ID # OR PROSPECTING tD # M-1997-027 <br />INSPECTION DATE 1-29-2003 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was a routine monitoring inspection conducted as part of DMG's ongoing monitoring program for permitted mines in <br />Colorado. Prior to the inspection, the operator had indicated a desire to close out this site. This report will attempt to address <br />remaining requirements of the reclamation plan. <br />The mine identification sign and affected area boundary markers are in place and in compliance with Rule 3.1.12. <br />No significant erosional features were noted on the affected areas of the site. <br />No significant infestations of noxious or other problematic weeds were noted on site during this inspection. The entire site has <br />been disturbed by mining leaving no indication of the previously noted knapweed patches. <br />The topsoil piles, though located right adjacent to the permit boundary, have not slumped across the boundary. These <br />stockpiles still do not support any stabilizing vegetation as noted in previous inspection reports. If reclamation, including <br />replacement of topsoil, is not completed by March 31, 2003, The operator must submit documentation that these piles have <br />had the surface loosened (tilled), that they have been seeded and mulched by that date. <br />A shop with office, deck, alternate access road, several concrete footer pads, propane tank, electricity, lawn, fuel tanks, and <br />water spigots have been located on the southeast corner of the site. These features are not addressed in the approved mining <br />or reclamation plans. The post mining land use for this site is rangeland. These features do not support this currently <br />approved post mining land use, though they appear to be permanent features. Therefore, the features must either be removed <br />from the site, or an amendment to the permit must be filed addressing a change of post-mining land use that will allow these <br />features to remain on site. This type of amendment application must include updated maps and text, and must request a <br />change of post-mining land use to either industrial/commercial or residential. An amendment will require all public notices, <br />publications, and submittal of an application package as was required for initial permitting ofthis site. An application form can <br />be requested from this office. If an application for amendment, addressing the above noted revisions, is not received by DMG <br />by March 14, 2003, and these features remain on site, an enforcement action may be taken. In addition to the enforcement <br />action, the reclamation cost estimate will be recalculated to include demolition and disposal costs for these features, increasing <br />the required financial warranty significantly. !n the future, the operator is strongly encouraged to contact DMG and revise the <br />permit appropriately prior to making unapproved revisions to the mine site. <br />There are two above ground fuel storage tanks located on the north side of the shop. These tanks are currently at or near <br />empty. Because -these tanks are not a current threat for spill of hazardous materials, this was not noted as a problem. <br />However, before these tanks are filled, a technical revision to this permit must be submitted that addresses installation of <br />adequate spill control structures for these and any other tanks to be stored on site containing hazardous materials. The <br />revision must also address changing the mine plan to include wording specified on the enclosed memo dated February 8, 2002. <br />The pit slope on the east side of the shop has been graded to a slope of 3:1 or less. It appears to have been topsoiled, but <br />vegetation has not been established here to stabilize the slope. If reclamation of the remaining portion of the site is to be <br />completed this spring, then this slope can be seeded with the remainder of the site.
