Mi~esite Inspection Narrative <br />Pays 3 of 3 <br />Eight culverts not approved in the mining and reclamation plan had been placed on <br />the site and had been discovered on a previous inspection. Notice of Violation (C- <br />93-48) had been issued covering this. At the time of the June inspection four of <br />these had been removed and the operator had submitted a revision requesting <br />approval fur the four other structures. The violation covering these was modified to <br />allow additional time for abatement. <br />A topsoil storage area not approved had been constructed just above the sediment <br />pund. This was covered by a Notice of Violation issued on a previous inspection. <br />with an abatement deadline of 6/25/93. <br />On a previous inspection the state had issued a Notice of Violation for the east <br />face of the refuse slope being steeper than allowed. This violation has an <br />abatement deadline of 7/2/93. <br />Drainage control on the Santa Fe #1 and Santa Fe #2 loadout was not functional <br />and nonexistent on the B&N loadout. Although sumps on the Santa Fe #1 and <br />Santa Fe #2 had been cleaned, drainage from all disturbed areas would not flow to <br />these structures and could flow off the permit area. No controls of any kind were <br />present at the B&N loadout and in fact the area had been recently plowed. As a <br />result the Division issued Notice of Violation C-93-87 cove.ng all three loadouts at <br />tha conclusion of this inspection. <br />The sediment pond for this site was not discharging during this inspection. Sumps <br />for the mine and loadouts contained little water although some runoff entered <br />structures as a result of rainfall on 6/9/93. <br />Notice of violation C-93-087 and Cessation Order CO-93-088 were hand served to <br />Alan Weaver, Engineer for Energy Fuels who arrived at the site at the conclusion <br />of the inspection. <br />
