• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-84-101 <br />INSPECTION DATE Auqust 22, 1997 INSPEC OR'S INITIALS TAS <br />The following problems (PB) and/or possible violations (PV) (and s gested corrective <br />actions) were identified during this inspection. The problems should e corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should e notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. The existing financial warranty of $5,000 is not <br />adequate to reclaim the 12.7 acres of disturbance to <br />rangeland. The Division estimates a financial warranty <br />of $18,000 will be required. <br />Please complete one of the following within 60 days: <br />2 <br />A. Submit an additional financial warranty of $13,000 <br />or a replacement warranty of $18,000. <br />B. Perform reclamation to the extent that the remaining <br />reclamation cost will be less than or equal to <br />$5,000. <br />The operator has begun construction of an asphalt batch <br />plant without within the 9.7 acre permit area prior to <br />approval by the Division. <br />Complete one of the following options within 30 days: <br />A. Submit an application for a technical revision to <br />the existing permit for operation of an asphalt batch <br />plant. Include a description of which portions of the <br />batch plant which are portable and which portions are <br />not. Include demolition costs and dimentions and <br />material type for portions which are non-portable. <br />B.
