• • (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-87-009 <br />INSPECTION DATE 6/17/98 INSPECTOR';: INITIALS AJW <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by ~.he Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There was not a mine identification sign posted at the 09/30/98 <br />public entrance to the mine site as required by Rule PB or PV: PB <br />3.1.12(1)(a),(b) & (c), and the affected area TOPIC(S): SM <br />boundaries were not marked by monuments or other <br />markers as required by Rule 3.1.12(2). <br />Corrective actions are to: <br />1) Place a mine identification sign at <br />the entrance to the mine site that <br />contains at a minium: the name of the <br />operator and "MLRB Permit # M-87- <br />009". <br />2) Mark the boundaries of the affected <br />area with markers that are clearly <br />visible and adequate to delineate <br />such boundaries. The location of <br />these markers should be based on the <br />new affected area map that will be <br />submitted with the technical revision <br />as described under #2. <br />3) Submit a photograph to our office <br />which clearly shows that the <br />identification sign and affected area <br />boundary markers have been installed. <br />2. The approved mine plan map does not match the current 9/30/98 <br />on-the-ground development for this mine site. PB or PV: PB <br />TOPIC(S): MP <br />Corrective Actions are to: <br />1) Submit a technical revision to the <br />application which clearly outlines <br />the proposed affected area and <br />matches the current mine development. <br />
