The following problems (PB) and/or possible violations (PV) (and suggested corrective
actions) were identified during this inspection. The problems should be corrected by the
dates given, or they will become possible violations. The possible violations should be
corrected by the dates given to reduce their severity when considered by ~.he Mined Land
Reclamation Board. The inspector noted on the previous page should be notified of all
corrective actions taken.

1. There was not a mine identification sign posted at the 09/30/98
public entrance to the mine site as required by Rule PB or PV: PB
3.1.12(1)(a),(b) & (c), and the affected area TOPIC(S): SM
boundaries were not marked by monuments or other
markers as required by Rule 3.1.12(2).
Corrective actions are to:
1) Place a mine identification sign at
the entrance to the mine site that
contains at a minium: the name of the
operator and "MLRB Permit # M-87-
009".
2) Mark the boundaries of the affected
area with markers that are clearly
visible and adequate to delineate
such boundaries. The location of
these markers should be based on the
new affected area map that will be
submitted with the technical revision
as described under #2.
3) Submit a photograph to our office
which clearly shows that the
identification sign and affected area
boundary markers have been installed.
2. The approved mine plan map does not match the current 9/30/98
on-the-ground development for this mine site. PB or PV: PB

Corrective Actions are to:
1) Submit a technical revision to the
application which clearly outlines
the proposed affected area and
matches the current mine development.
