(Page 2i <br />MINE ID # OR PROSPECTING ID # M-1998-080 <br />INSPECTION DATE 9/5/02 INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This inspection occurred as part of DMG's routine monitoring of permitted operations. The H & <br />H Quarry No. 2 permit is approved for 9.9 acres affected area. DMG holds 56,347 financial <br />warranty. Permit status is active. <br />Mine identification sign, required by Rule 3.1.1211), was not observed. Rule 3.1.12(1) states that <br />the operator shall post a sign at the entrance to the mine site, clearly visible from the access road <br />and which displays the name of the operator, the permit number, and a statement that a <br />reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation <br />Board. <br />Permit boundary was well delineated with fence post and survey ribbon. Mining and reclamation <br />activities appeared to be confined within the approved permit area. Several areas of excavation <br />and quarry activity have occurred within the permit area, depending upon the nature and quality <br />of material encountered. Near surface sandstone, highly weathered and relatively uniform in <br />fracture pattern, was being excavated from-the southern portions of the permit area. Much of the <br />highly weathered material could be removed from the pit floor by hand and palletized for transport. <br />Quarry activity at the northern portions of the permit area appeared to have encountered more <br />competent, less weathered material which must 6e drilled and split from the quarry face. The <br />massive sandstone blocks, removed from the northern portions of the permit area, have resulted <br />in a benched highwall with 2 benches, each bench approximately 5' high and 10' deep. The <br />configuration of the highwall in the southern portions of the permit area was approximately 350' <br />long with portions of the highwall approximately 8` near vertical rock face with approximately 11' <br />of sloped overburden behind the crest of the highwall. The configuration of the highwall in the <br />northern portions of the permit area was approximate'ly'300' long with approximately 15' of sloped <br />overburden behind the crest. Topsoil and overburden, salvaged from the initial surface <br />disturbance, was stockpiled above the advancing face of the highwall. Various stockpiles of <br />overburden, waste rock and mined material were located on the pit floor. Total affected area was <br />visually estimated at 6 acres. <br />The approved reclamation plan indicates that affected lands will 6e reclaimed to support wildlife <br />habitat post mining land use. The highwall, final height anticipated at 25' high, will be either <br />stabilized with a series of benches, each bench approximately 5' high and 10' wide, OR reduced <br />to 3H:1 V slope by backfilling. Large sized waste rock material will be relocated from the pit floor <br />to the base of the highwall, Available overburden and'small waste rock material will be distributed <br />over the pit floor prior to topsoil replacement. Pit floor will be ripped to 2' depth prior to topsoil <br />replacement. Topsoil will be replaced to 6"-8" depth. Vegetative cover will be established on the <br />7.7 acres portion of the permit area administrated by the BLM, by planting Indian ricegrass, Sand . <br />dropseed and Sainfoin. Vegetative cover will be established on the 2.2 acres portion of the permit <br />area owned by Richard Perkins, by planting Crested wheatgrass and Sainfoin. <br />
