(Page 2)
MINE 1D # OR PROSPECTING ID #: nn-r a9n_n4a
INSPECTION DATE: L2/IdrnS INSPECTOR'S INITIALS: ~
OBSERVATIONS
This inspection was conducted as part of the Division of Minerals and Geology's four-year monitoring
program. Present at the inspection were Kate Pickford, DMG, and Marcia Talvitie, Four Corners
Materials. Weather at the time of the inspection was clear, calm, and approximately 40 degrees.
2. A sign is posted at the entrance to the site and the permit boundaries are easily identified in the field.
Four Corners Materials currently has a lease with the permit holder to mine the site. There is presently no
active mining at the site due To the location of the site relative to market locations. According to
Division records, no mining activity has occurred of the site since sometime prior io December of 2002.
In that Four Corners has no current intention to commence mining at the site, the site should be put into
Temporary Cessation status (see problem 1, page 3 of this report).
3. The site consists of a lower area where access to the site is gained and where scales are located and
some stockpiles of material are stored. Access to the active mining area is gained via a roadway from
the lower area, up the mesa, to the top. Disturbance on the top of the mesa consists of an excavation
and stockpiled overburden and topsoil, northwest of the pit, There is also an access or processing area.
The disturbance does not exceed the 8 acre limit stated in the mining plan. The excavation has
exposed, vertical highwalls, which fal( into the parameters of the approved mining plan. The disturbed
area outside of the excavation is surrounded by containment berms along the edge of the mesa.
4. The mining plan for the site states that if the stock piles of topsoil and overburden are left undisturbed for
over one year, both will be seeded to prevent erosion. There was no evidence of seeded vegetation
on these stock piles at the site during this inspection (see problem 2, page 3 of this report).
5. Based on a partial bond calculation conducted as a part of this inspection, the currenfi bond amount of
$72,645.00 appears to be adequate for reclamation of the site.
18 E Contact Address
NAME: Dale L Trenkle
OPERATOR: Hiah Flume Gravel Pit.
