The purpose of this revised inspection report is to correct the error from the 12/1/99 <br />report. Corrected errors include the permit number on page 1 and description f the Problem (PBI, <br />for insufficient financial warranty, on page 4. Corrective action deadline for ih financial warranty <br />is sixty (601 days from the date of this corrected inspection report. <br />This inspection occurred as part of DMG's routine monitoring plan for permitt d operations. Jim <br />Helmericks accompanied the DMG inspector throughout the inspection. Ope ations were active <br />during the time of this inspection. The J & J Pit is approved for 9.9 acres a ected area. DMG <br />holds 55,000 financial warranty. <br />Signs & Markers Mine identification sign was posted at the mine entrance r ad. Mine entrance <br />road was gravel surfaced and well maintained. Affected area perimeter ma kers were posted. <br />Mining activities were confined to approved areas. <br />Affected Areas Mining equipment included material screens and convey rs, dozer, loader, <br />backhoe, trucks and truck scale. The main pit daylights to the west and incl des approximately <br />2 acres affected area. Portions of the 30' tall highwall contained near verti al sections, overall <br />slope of the highwall approximated a 1 H:1 V slope. A second pit area was I Gated immediately <br />east (upslope) of the main pit area. The second pit area included approximat ly 3 acres affected <br />area, 10' tall highwall approximated a 1 H:1 V slope. Material stockpiles appear d to have affected <br />an additional 2 acres, total affected area was approximately 7 acres. Affec ed areas appeared <br />stable, evidence of excessive erosion and/or sedimentation to affected areas an surrounding areas <br />was not observed. <br />Reclamation Plan The approved reclamation plan indicates that recta ation will support <br />industrial/commercial and/or residential post mining land use. Pit highwalls will be reduced to <br />2H:1 V slope and stabilized with transplanted Pinion pines and seeded wit Indian ricegrass, <br />Intermediate wheatgrass, Western wheatgrass, Needle and thread grass, and M untain mahogany. <br />Pit floor with be gravel surfaced, not revegetated, and graded approximat ly flat. The mine <br />entrance/haul road existed prior to the permit application and will remain as a permanent access <br />road. Mining related structures, such as the truck scale and scale house, wit be removed. <br />Notice to Increase the Financial Warranty DMG has completed a preliminary re lew of the current <br />cost of reclamation and has determined that the existing 55,000 financial war anty is insufficient <br />to ensure reclamation. DMG current reclamation cost estimation totaled S ,198. Please find <br />enclosed, twelve (12) pages of summaries, worksheets and data sheets, utilized by DMG to <br />determine the cost of reclamation. DMG reclamation cost estimation is based pon conditions of <br />the approved reclamation plan and observations made during the inspection Pursuant to Rule <br />4.2.1(21, the operator has sixty (60) days to post 51,198 additional financial arranty, to ensure <br />a total financial warranty not less than 56,198. If the operator disagrees I ith this notice to <br />increase the financial warranty, DMG shall schedule the matter for a hearinci before the Board. <br />Please ensure that DMG has received not less than 56,198 financial warrantv1by April 14. 2000. <br />
