• (Page 3) <br />MINE ID # OR PROSPECTING ID # M-05-010 <br />INSPECTION DATE 1/24/96 INSPECTOR'S INITIALS WHE <br />The following problems (Pal and/or possible violations IPV) land suggested corrective actions/ were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Mine identification sign and affected area perimeter markers were not observed. Rule 3.1.1211) requires that the operator post <br />a sign at the entrance to the mine site, clearly visible from the access road, which displays the name of the operator, the permit <br />number, and a statement that a reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation <br />Board. Rule 3.1.12(21 states that the boundaries of the affected area will be marked by monuments or other markers that are <br />clearly visible and adequate to delineate such areas. <br />CORRECTIVE ACTION: Post and maintain mine identification sign and affected area perimeter markers required by Rule 3,1.12. <br />Sign and markers shall be maintained until OMG approves complete release of performance warranty and termination of permit. <br />2129/96 <br />PB or PV PB <br />' TOPICIS) SM <br />2. Exploration holes, excavated prior to permit issuance, remain open and are a potential threat to health and safety. Exploration <br />holes should be backfilled and reseeded, Location of exploration holes shall be identified on Mining Plan Map and within the <br />Reclamation Plan, as required by Rule 6.3.51211f1. <br />CORRECTIVE ACTION: Backfill exploration pits, reseed with approved reclamation seed mix during spring seeding season. <br />Submit a Technical Revision to the permit, as described in Rules 1.1149) and 1.9, containing updated Mining Plan Map and <br />Reclamation Plan to identify all existing disturbance within and/or adjacent to the permit boundary, as required by Rule 6.3.512)If). <br />Section 34-32.5-125 of the Colorado Land Reclamation Act for the Extraction of Construction Materials, 34-32.5.101, et seq, <br />C.R.S., 1995, as amended, states that there is no fee associated with applications for Technical Revision for 110 permits. <br />2/29196 <br />PB or PV PB <br />70PICIS) BG <br />MP, RS, __ <br />3. Total affected area appeared to be limited to the 9,9 acres approved to this permit; however, disturbance has shifted south and <br />east from the location depicted on hand drawn maps originally submitted with permit application, Mining Plan Mep and <br />Reclamation Plan Mep need to bs updated to accurately outlirts and label the permit boundaries, as required by Rule 6.3.51211x1. <br />CORRECTIVE ACTION: Submit a TschNcal Revision to the permit, es described in Rules 1.11491 end 1.9, containing updated <br />Mining Plan Map end Reclamation Plan Mep, which specifically sddrasa the requirements of Ruls 6.3.512i(a- es well ae the general <br />requirements of Rule 8.3.5.
