The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />1. Annual reports, submitted by the operator, indicate that operation has been dormant for several <br />years. However, operator has not filed Notice of Temporary Cessation, as required by Rule 1.13.5. <br />CORRECTIVE ACTION: Submit Notice of Temporary Cessation, in accordance with Rule 1.13.5, <br />for past years of non-production and cessation of mining operations. If operator desires to revise <br />mine status to allow for intermittently active operations, submit Substitute for Notice of Temporary <br />Cessation, as provided by Rule 1.13.7. Submit appropriate notice by October 27, 1997. <br />10/27/97 <br />PB or PV PB <br />TOPIC(S) AR <br />-' - - <br />2. Mine identification sign, required by Rule 3.1.12(1-, was not observed. Rule 3.1.1211) states <br />that an identification sign will be posted at the entrance to the mine site which displays the name <br />of the operator, the permit number, and a statement that a reclamation permit has been issued by <br />the Colorado Mined Land Reclamation Board. Affected area perimeter markers, required by Rule <br />3.1.1212), was not observed. Rule 3.1.1212) requires that the permit boundary be delineated by <br />monuments or markers. <br />CORRECTIVE ACTION: Ensure mine identification sign and affected area perimeter markers are <br />posted by October 27, 1997. <br />10/27/97 <br />PB or PV PB <br />TOPIC(S) SM <br /> <br />CERTIFIED MAIL NO.
