(Page 3) * • MINE ID # OR PROSPECTING ID # M-78-319 INSPECTION DATE 11/17/00 INSPECTOR'S INITIALS ESC The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLBMS/POSSIBLH VIOLATIONS AND CORRBCTIVB ACTIONS 1. Boundary markers have only been partially posted at the Hewes Pit. CORRECTIVE ACTION: Post additional markers at the site and submit proof in the form of photographs by February 1, 2001. 2. The operator submitted revised maps of the site as required during the Division's June 24, 1996 inspection, but were not submitted in the form of a revision to the permit. CORRECTIVE ACTION: The operator will need to submit a letter to the Division requesting these maps (dated August 3, 1998) be included in the Mining and Reclamation Permit for the Hewes Pit. The Division will waive the $188.00 application fee since the maps were received by the Division prior to the adjustment of application fees for 110 operations. PB or PV:PB C.R.S. 34-32.5-117(4)(c)(I)(a) states that the Board may increase TOPIC(S):FN the amount and duration of a required financial warranty to reclaim Februarv 1, 2001 the site. The Division's reclamation cost estimate based on the site conditions and permit requirement totals $8,000.00. CORRECTIVE ACTION: The operator has the following options; • Post an additional $7,000.00 bond by February 1, 2001, • Complete some reclamation on site, or • Revise the reclamation plan.
