The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMSlPOSSIHLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br /> CORRECTION <br /> DATE <br />1. A portion of the pit has been excavated outside the 11-20-95 <br />southern permit boundary. PB or PV PV <br /> TOPIC(S) MP.OD <br />Corrective Action: Submit to the Division a complete <br />conversion application which includes all disturbed <br />areas which are currently outside the permit no later <br />than 11-20-95. Appear before the Mined Land <br />Reclamation Board at the October 18-19, 1995 meeting. <br />2. The financial warranty amount of $ 5,000.00 is 11-20-95 <br />insufficient for this permit. The Division has re- PB or PV PB <br />calculated the required amount to be $20,200.00. TOPIC(S) FN <br />Corrective Action: Post a bond for $20,200.00 or submit <br />a rider to the current bond no later than November 20, <br />1995. <br />3. A wash plant is used in this operation. This ie not 11-20-95 <br />currently included in the permit. PB or PV PB <br /> TOPIC(S) SF <br />Corrective Action: Include in the conversion <br />application required for PV-1, a description of the <br />wash plant and the cost to reclaim it. <br />
