The following problems (PB) and/or possible violations (PV) (and suggested corrective
actions) were identified during this inspection. The problems should be corrected by the
dates given, or they will become possible violations. The possible violations should be
corrected by the dates given to reduce their severity when considered by the Mined Land
Reclamation Board. The inspector noted on the previous page should be notified of all
corrective actions taken.

1. The Division was unable to locate a mine identification sign on _
site. Rule 3.1.12 requires a sign be posted clearly visible PB or PV:PB
from the access road with the name of the operator, a statement TOPIC(S):SM
that a reclamation permit for the operation has been issued by Julv 1B, 2000
the Colorado Mined Land Reclamation Hoard and the permit number.
CORRECTIVE ACTION:
The operator must post a mine identification sign clearly
visible from the access road with the name of the operator, a
statement that a reclamation permit for the operation has been
issued by the Colorado Mined Land Reclamation Board and the
permit number. Photographs of the posted sign must submitted by
July 16, 2000 demonstrating compliance.
2. A permit overlap exists between Quarry E (Permit No. M-86-115)
and the Hector Rodriguez Quarry (Permit No. M-99-062).
CORRECTIVE ACTION:
BMS and Hector Rodriguez will need to clarify this discrepancy
in the respective permits by July 18, 2000. The operator that
relinquishes the 0.5 acres will need to request a bond release
for that area to permanently remove it from the permit area.
