(Page 2) • <br />• <br />MINE ID # OR PROSPECTING ID # M-99-062 <br />INSPECTION DATE OS/11/00 INSPECTOR'S INITIALS ESC <br />OBSERVATIONS <br />This was an inspection of the Indio Red Quarry conducted by Erica Crosby of the Division of <br />Minerals and Geology. Hector Rodriquez, the operator and permittee was present during the <br />inspection. Weather conditions on site were clear and ground conditions were dry. <br />The 110 Mining and Reclamation Permit was issued to Hector Rodriquez on August 23, 1999 to <br />mine flagstone on a permit area of 9.9 acres. The site is divided into two quarries; the <br />Indio Red Quarry #1 and #2. Indio Red Quarry #1 is located directly east of the Gaspars <br />Quarries (Quarry E) Permit No. M-86-115 operated by Blue Mountain Stone (" BMS" ). <br />Quarry #1 was surveyed using the Global Positioning System (" GPS" ) due to its proximity to <br />Quarry E. The Indio Red Quarry #1 measured 5.2 acres in size, which is consistent with the <br />approved mine plan map. Based on the GPS survey taken during the inspections of 5/10/00 and <br />5/11/00, there appears to be a 0.5-acre permit overlap between the Indio Red Quarry #1 and <br />Quarry E of Permit M-86-115. Please see attached map for further clarification. The <br />Division does not allow two different operators to hold two separate mining and reclamation <br />permits on the same parcel of land. Only one permit can cover the 0.5-acre area. BMS and <br />Hector Rodriguez will need to clarify this discrepancy in the respective permits. The <br />operator that relinquishes the 0.5 acres will need to request a bond release for that area <br />to permanently remove it from the permit area. If it is to be added elsewhere in an existing <br />permit, a permit amendment or 112-conversion application may be required. <br />Indio Red Quarry #2 is located northeast of the Red wolf Quarry, Permit No. M-99-063. <br />Quarrying is taking place in this area, but is very limited in disturbance. Quarry #1 is <br />more actively being mined, but is no more than 2 acres in size. <br />Permit boundary markers were posted to delineate the permit area. The markers appear to be <br />consistent with the approved mine plan map. The Division was unable to locate a mine <br />identification sign on site. Rule 3.1.12 requires a sign be posted clearly visible from the <br />access road with the name of the operator, a statement that a reclamation permit for the <br />operation has been issued by the Colorado Mined Land Reclamation Board and the permit number. <br />See page 3 for compliance due dates. <br />I ~ E Contact Address <br />NAME Hector Rodriouez <br />OPERATOR <br />STREET P.O.
