Mr. Steve Shuey <br />Re: Monogram Mesa Permit M-1978-222-UG <br />Page 2 of 3 <br />Pursuant to a later inspection by the Division, a mine site identification sign was posted at the <br />Great Hesper Pit and notification was sent to the Division on November 6, 2002. <br />The matter of boundary markers remained open due to the necessity for the Division and <br />Operator to identify and agree upon which sites were the ongoing reclamation obligation of the <br />Operator. This matter is no longer pertinent as the one site that is to be reclaimed (the Great <br />Hesper Pit) by the Operator is in final reclamation. <br />November 27, 2001 Inspection bV Steve Shuev: <br />Previous Corrective Actions pertainine to site reports, identification signs, and boundary <br />markers: Inspection conducted to verify compliance with outstanding Corrective Actions. The <br />Division was unable to identify, during the November 27 inspection, a number of the sites listed <br />under the Permit due to recent snowfall. The Division proposed joint site examinations with the <br />Operator to resolve the proper identification of sites, determine which sites need to have updated <br />maps, to observe reclaimed azeas that might be releasable from bonding coverage, and to <br />determine which sites might need to have boundary markers. <br />Verification of the adequacy of the submitted materials on reported site conditions was reserved <br />until a joint site examination could be scheduled. <br />November 29 Response from IUSA: <br />Submittal of site reports and maps: The Operator provided photographs and site descriptions of <br />those individual sites which were included in the original Permit. Maps were also provided <br />showing the approximate locations of the sites which could be positively identified in the field. <br />These materials were submitted in partial fiilfillment of the outstanding Corrective Action items. <br />July 17, 2002 Inspection by Steve Shuev: <br />Records: Historical documentation was provided that confirmed that only the Great Hesper Pit <br />site was disturbed under the Permit during the period from Permit issuance up to 1986. Annual <br />Reports from 1986 forwazd confirm that no other mine sites were disturbed under the Permit. <br />No other sites listed under the original Permit are required to be reclaimed by the Operator, <br />although the Division strongly recommends that hazardous sites be reclaimed and that weed <br />infestations be addressed. <br />S=~ Amine site identification sign was requested for the Great Hesper Pit. <br />Boundary markers: The Great Hesper Pit is in final reclamation, and therefore Affected Area <br />boundary markers are not required. <br />Seedine, settline, rillin>;, debris/trash at Great Hesper site: A number of site observations were <br />provided by the inspector.
