' ~ (Page 2) <br />MINE ID # OR PROSPECTING ID # M-89-033 <br />INSPECTION DATE 6-18-96 INSPECTOR'S INITIALS GSC <br />OBSERVATIONS <br />This was a monitoring inspection conducted with Mr. Stevan O'Brian of Environment, Inc. and <br />Mr. Francis Romik of Albert Frei & Sons, Inc. Mr. Albert Frei was present during the <br />closeout meeting. Observations are as follows: <br />1. The operator currently buys water from the City of Idaho Springs for dust control. <br />Application has been made with the Division of Water Resources for a substitute supply plan <br />to use groundwater from wells in the pit. <br />2. The runoff water discharge point is located by the scale house. A sediment trap is <br />located near this point. During the inspection, a loader was observed cleaning out the trap. <br />3. Newly acquired loadout bins are being installed in the process area. <br />4. Mining refuse was present in the Stage I process area, especially in the vicinity of the <br />fuel tanks. All refuse needs to be disposed of pursuant to Rule 3.1.5(5). See Problem PB-1. <br />5. Stage Y is used as an equipment parking area and as a staging area for various equipment <br />and materials. A crew was observed loading refuse into a dumpster in this area. <br />6. According to the operator's representative, Stage X is used periodically to store fines. <br />During this inspection, a veneer of fines covered the eastern side of the stage near Clear <br />Creek. A berm has been placed at the base of the terrace along Clear Creek to catch fines <br />which have eroded down this embankment and the slopes of the terrace have been stabilized <br />with rip rap. Some of the erosion control berm running along the eastern top edge of the <br />terrace has been removed, however, and this needs to be reestablished and stabilized. A <br />stockpile of fines is located in Stage Z for reclamation and this also needs to be stabilized <br />or graded and seeded. See Problem PH-2. <br />7. Above ground fuel tanks and oil drums are stored in an area without a spill containment <br />structure. They are located in the western side of the pit. The tanks include a 5,000 gal. <br />(approx.) fuel tank, two 200 gal. gasoline tanks, and a half dozen oil drums. These need to <br />be enclosed within a spill containment structure.
