<br />Energy, Inc. The period of coverage is 5-26-92 to 5-26-93. The <br />limits are $500,000,00 for each occurrence and $1,000,000.00 <br />general aggregate. The accord form which indicates the insured, <br />certificate holder, operation insured and cancellation notice was <br />not at the mine at the time of the inspection but was provided by <br />the Division of Minerals and Geology (DMG) the following week. <br />Ownership and control: A comparison of the Applicant Violator <br />System (A VS) information with the current permit information <br />found that John Prior is no longer a shareholder in Twin Pines <br />Investment. This was confirmed by mine personnel. AVS does not <br />indicate a permit has been issued to Twin Pines Investment <br />although it does indicate an application was filed. DMG needs to <br />update the information contained in the system. <br />Bond: The bond is secured through Certificate of Deposit #581898 <br />issued by Fremont National Bank of Canon City. The certificate <br />is automatically renewable and is in the amount of $54,130.00. <br />This amount agrees with the reclamation cost estimates. There is <br />an additional bond in the amount of $989.00 in the form of a <br />Certificate of Deposit, #3705. This bond was required due to <br />minor revision #5 which allowed .25 acres to be disturbed for <br />exploratory drilling. <br />Blasting: No surface blasting is being done at the mine. <br />NPDES or discharge permit: Discharge permit #CO-0037541 has been <br />issued by the Colorado Department of Health for operations at <br />this site. 'The permit has an expiration date of 10-31-95 and <br />covers 4 discharge points at the mine and associated loadout. <br />On 1-24-91 the Department of Health notified the mine that a <br />discharge in excess of pezmits limits occurred at outfall number <br />002A. <br />Water monitoring: Other than discharges there are two wells which <br />are monitored for water quality and quantity. Theae wells are <br />monitored quarterly and the data is submitted semi-annually. The <br />monitoring information was submitted to DMG as required. <br />5 <br />
