(Page 3)
NINE ID # OR PROSPECTING ID #
INSPECTION DATE June 11. 1996
M-77-057
PROBLEMS/POSSIBLE VIOLATIONS
AND
CORRECTIVE ACTIONS

INSPECTOR'S INITIALS
CORRECTION
DATE
1. An identification sign was not posted at the entrance
to the mine site which contained the mine name, a
statement that CMLRB had issued the permit, and the
mine permit identification number, as required by rule
3.1.12(1). In addition, the boundaries of the affected
area were not marked by monuments which were clearly
visible and adequate to delineate such boundaries as
required by rule 3.1.12(2).
The operator should address both of these items by
placing a mine identification sign at the entrance to
the mine which includes the mine name, a statement that
CMLRB issued the reclamation permit, and the permit
number. The second part of this corrective action ie
to place permit boundary markers (such ae painted T-
poets) along the perimeter of the permit area which
will clearly delineate the affected area. Upon
completion of these activities and prior to expiration
of the corrective action date (8/20/96), the operator
shall submit a photograph(s) which clearly shows that
the I.D. sign and boundary markers have been installed.
Two noxious weeds, Canada thistle and Dalmatian
toadflax, were observed on Bite. It did not appear
that any efforts have been extended to control these
weeds ae required under rule 3.1.10.
Therefore, the operator shall control both of these
weed species and any other noxious weeds through the
appropriate chemical, mechanical or biological control
method. Upon completion of these activities and prior
to the expiration of the corrective action date, the
operator shall submit another photograph(s) that
clearly shows appropriate weed control methods have
been employed to combat the aforementioned noxious
weeds.
8/20/96
PB or PV PB
TOPIC(S) SM
CORRECTION
DATE
8/20/96
PH or PV PB
TOPIC(S) RV
AJW CBM
